Reporting a Safety Concern
Patients and families are encouraged to ask questions and share concerns about care or safety at any time.
If you have a concern:
Speak with a member of your care team as soon as possible.
If you don’t reach a resolution with the team providing your care, we encourage you to contact the department manager, charge nurse or supervisor.
If the attempts above are unsatisfactory, please contact Patient Advocacy at the hospital where care was received. Contacts for patient advocates at your hospital are provided below.
In urgent or emergency situations, please seek immediate assistance from a care team member.
Contact a Patient Advocate
If you have questions, concerns or feedback about your care experience, our Patient Advocates are here to help. Patient Advocates serve as a resource for patients and families by listening to concerns, helping resolve issues, clarifying hospital policies and ensuring your voice is heard.
We are committed to respectful, compassionate care and open communication across HH Health facilities. If something does not meet your expectations, we encourage you to reach out so we can address it promptly.
What Patients Can Do to Stay Safe
Patients and caregivers play an important role in supporting safe care. You can help by:
- Asking questions if something is unclear
- Sharing a complete list of medications and allergies
- Confirming your identity before procedures or treatments
- Letting your care team know if something doesn’t feel right
Your voice matters, and we encourage you to be an active partner in your care.
